Monthly, listing each session by date, is easiest for parents and for your own records. Per-session invoices work for occasional students. Either way, every session needs a date and a length on the invoice.
If the parent agreed to a cancellation policy beforehand, yes. Print the rule on the invoice (“under 24 hours, 50%”), and when you apply it, put the date and the words “per terms” on the line.
Invoice the package when it is bought, then on each monthly invoice list the sessions as usual and add a credit line for the hours used from the package, with the hours remaining noted.
Private tutoring is not subject to sales tax in most states. Your income is taxable; parents may use the invoice for dependent care or education accounts, which is why your tax ID and the dates matter.
