Your details, the client's details, an invoice number and date, one or more lines with amounts, a total, a due date and how to pay. This template is exactly that; everything else is optional.
For new clients, yes: 30–50% before work starts, shown as a credit on the final invoice. It filters out clients who were never going to pay.
