If you bill monthly for four visits, list the four dates. One line “Cleaning — $480” looks like a mistake to a client who remembers one visit.
If you bring supplies, say “supplies included”. If the client supplies them, say that too.
Lines a typical job produces. Labor and materials are separate, and anything already paid is a negative line so the total is the balance due.

Residential clients usually prefer per visit, paid the same day. Offices and landlords prefer one monthly invoice listing each visit date. The template works for both.
A short fixed menu on the invoice (oven $35, fridge $30, windows $4/pane) means you can add them in seconds and the client already knows the price.
Depends on the state. Residential cleaning is taxable in some states (for example Texas and New York) and exempt in others. Check your state; the template has a tax line you can set to 0%.
Put a cancellation policy in the terms (“24 hours' notice or 50% of the visit”). A policy printed on every invoice is enforceable; one mentioned once in a text is not.
