Hours, rate and what was done in those hours. The client is buying time they did not watch, so the invoice has to make the time visible: which days, how many hours, on what. An invoice that says “Labor, 14 h, $840” gets a phone call; one that says what the 14 hours were gets paid.

One line per day or per task with the description, hours and rate (“Mar 3, demo and prep, 6.5 h × $85”), materials on separate lines, the total, and your payment terms. Attach a timesheet if there are many entries.
To the nearest quarter hour is standard in the trades, six-minute increments in professional services. State the rule on your estimate and apply it the same way every time.
Yes if it was agreed before the job. Bill it as its own line at the agreed rate or flat fee; do not fold it into the work hours.
Enough that the client recognizes the work: the day, the task, and anything unusual. “Troubleshoot breaker tripping, found damaged wire in attic, replaced 14 ft” is paid; “electrical work, 3 h” is questioned.
Not for a few entries that fit on the invoice. For retainers, long projects or clients who approve hours, a timesheet attached to the invoice is expected and protects you.
