If Saturday is $28 and Tuesday is $25, say so on each line, or the invoice looks inconsistent.
Long-term care insurers need the care recipient's name, the dates and hours, the type of care (personal care, companion care) and the caregiver's name and credentials. Missing fields delay the claim by a billing cycle.

The care recipient's name, each date of service with hours, the type of care provided (personal care, homemaker, companion), the rate and total, and the caregiver's or agency's name, address and credentials. Many insurers also require a care plan on file and daily notes; the invoice is the billing half of that record.
Separate the sleeping overnight (a flat rate for the night with the hours stated) from the awake overnight (hourly at a night rate). Name the type on the line so the payer sees why the two nights differ.
