This template has those fields in the header so a normal session invoice also works as a superbill.

Provider name and license number, date of service, the service description with length, the amount, and usually a CPT code (97124 therapeutic massage, 97140 manual therapy, used where appropriate). Some plans also require a letter of medical necessity from a doctor; the invoice cannot replace that.
In some states and with some plans, especially for auto-injury (PIP) claims and workers' compensation, with a referral and the right codes. The invoice then functions as a superbill: add the client's date of birth, the referring provider and the diagnosis code on the client's documents.
Invoice the full package when it is purchased. For each session afterward, issue an invoice showing the session with a credit line (“package session 3 of 5, $0 due”) so the client has a reimbursable record per visit.
As an optional separate line, never folded into the service price. It keeps the service fee reimbursable and separates tip income in your books.
Services by a licensed therapist are not subject to sales tax in most states; a few tax massage unless it is medically prescribed. Retail products sold to clients are taxable. Check your state.
