If you work for yourself without a company name, your invoice is still a business document and the client's bookkeeper will treat it as one: they need a name that matches your W-9, a tax ID or a note that it is on file, an invoice number, and lines that say what was done. This template is built for a person, not a brand: your name in the header, a tax-ID field, and service lines that work for consulting, trades, creative work or labor.
Free, fillable, US Letter. No email, no sign-up. Fill in your name, address and tax ID once. For each client change the lines and the terms.
What the PDF looks like. The hints in the line table show the lines a self-employed person typically bills; type over them.
The invoice says “Jake's Carpentry”, the W-9 says Jake Morrison. AP holds it. Use your legal name or register the DBA and use it on both.
Businesses cannot pay an unnumbered invoice through their system. Start at 1001 or 2026-001.
Mileage, materials and software reimbursements go on their own lines with receipts, so the service fee is clean for the 1099.
Lines a typical job produces. Labor and materials are separate, and anything already paid is a negative line so the total is the balance due.

Yes. A sole proprietor invoices under their legal name; a DBA can be added once it is registered. What matters is that the name on the invoice matches the name on your W-9 so the client's 1099 is issued correctly.
No. Get a free EIN from the IRS and use that; clients need a tax ID on the W-9, not necessarily on the invoice, but having the EIN on the invoice saves a request. Never put your SSN on an invoice that will be emailed around.
Your name matching the W-9, a tax ID or a W-9 on file, a unique invoice number, the client's PO number if they issued one, dated service lines, the total and payment terms. Missing any of these is the usual reason a company's AP department holds payment.
Separate lines for materials, mileage and other reimbursables at cost, with receipts attached or available. Keeping them apart from your fee keeps the 1099 clean and makes your service rate visible.
Only if your state taxes the goods or services you sell and you are registered to collect. Most services are not taxed; materials you resell often are. Check your state; the business structure does not change the answer.
