The provider's name, address and taxpayer identification number (EIN or SSN), the amount paid during the year, and ideally the child's name and the dates of care. Parents enter the first three on IRS Form 2441; the rest is what their employer plan or a state subsidy checks.
If you are a sole proprietor without an EIN, parents can request your SSN on Form W-10 and you must provide it. Getting a free EIN from the IRS takes ten minutes online and lets you put the EIN on receipts instead.
Issue one for every payment, because parents reconcile their employer accounts monthly, and then a single year-end statement in January that totals the year. Both come from the same template.
