In many states yes, at least for cash: for example California, Maryland, New York, Washington and Massachusetts require a written receipt when rent is paid in cash, and some require one on request for any payment method. Even where it is optional, issuing a receipt every month protects you more than the tenant.
Yes, and you should. Write the amount actually received, the period it applies to and the balance remaining with its due date. Accepting a partial payment does not waive the rest unless the receipt says so.
The bank record proves the transfer, but it does not say which month or which unit. A receipt does. Tenants applying for assistance or filing taxes are regularly asked for receipts, not bank screenshots.
Date, tenant, address and unit, rental period, amount in figures, the words “paid in cash”, your name and signature. Keep a copy in the tenant file; it is the only record either of you will have.
An invoice asks for money and shows what is due. A receipt confirms money was received: how much, when, how it was paid and what it paid for. Send the invoice first, the receipt after payment, and keep both.
