What is a proof of delivery receipt?

A document signed by the recipient confirming what arrived, when, and in what condition. This template is one; the signature and condition lines are what make it proof rather than just a charge receipt.

Do I need the recipient's signature on a delivery receipt?

For proof of delivery, yes. If nobody is home and the goods are left, note where and photograph them; write “left at door, photo on file” on the signature line.

Should the delivery receipt show the price of the goods?

Only if you sold the goods as well. A courier delivering for a third party lists the items and the delivery charge, not the merchandise value, unless the shipper asks for it for insurance.

What if the client paid part of the amount?

Issue a receipt for the amount actually received and show the balance remaining on it. When the rest arrives, issue a second receipt that references the first.
